Description

PRIMARY FUNCTION:

Provide professional, comprehensive, family oriented nursing care to individuals in the home setting. Assess patients' and families' needs and formulate, implement and evaluate plan of care. Perform primary case management function of assigned patients.

WORKING PROCEDURES AND/OR RESPONSIBILITIES:
  • Assess, monitor and intervene when appropriate with respect to the patient's physical and emotional needs. Develop appropriate goals with patient and family.
  • Develop and coordinate accurate written plans of treatment with physicians and others involved in the care of patients, utilizing appropriate care plans and protocols. Update care plan to reflect change in physician orders or patient condition and evaluate at least every 60 days and at discharge.
  • Provide nursing care in accordance with physician's orders, VNA protocols and procedures, nursing standards and sound public health principles with respect to infection control.
  • Notify physician of changes in patient's condition and care needs and obtain new telephone orders in an accurate and timely manner.
  • Perform all follow-up communication tasks relating to direct patient care, such as calls to physicians, arrangements for support services and transportation to appointments, as indicated.
  • Participate in multi-disciplinary team conferences and document outcome. Refer to other disciplines as necessary. Seek consultation from supervisor, physician and other providers as indicated.
  • Maintain documentation in patient record per internal, professional and regulatory standards. Complete additional updates as required.
  • Assist with agency or community relations projects as delegated by supervisor.
  • Meet the criteria for continued employment by maintaining current RN license, current CPR certification, attendance at quarterly in-services, current drivers license and auto insurance, and compliance with VNA policies. Maintain productivity requirement.
  • Maintains the highest ethical standards of business practice and adheres to all local, state and federal laws and regulations, quality standards and policies and procedures as directed by the Corporate Compliance Plan.
  • Under the federal privacy rule, uses, discloses and/or requests the minimum necessary amount of protected health information to effectively perform the job functions or achieve the purpose of the disclosure/requests.
  • SCOPE, PURPOSE, FREQUENCY OF CONTACT:
  • Internal Contacts: Exchange information relating to patient care with all levels of employees in various departments. Daily
  • External Contacts: Exchange information relating to patient care with physicians, caregivers, social workers, and other health care providers. Daily
  • DIRECTION TO OTHERS:

    Provides direction to HHA staff.

    DIRECTION RECEIVED:

    Receives direction from the Clinical Manager.

    Qualifications

    EDUCATION REQUIREMENT:

    Current license as RN in state of Maryland.

    EXPERIENCE REQUIREMENT:

    Traditional: Two years of medical/surgical nursing experience mandatory.
